What kind of proof do you need that you have taken an STD test and have passed it?  What documentation do you need?

You need to go to your doctor and tell him that you need a full HIV and STD test. He will address you to the right place where to have you checked, they will release you a certification with your name and the date when you took the test in it and then you will send a scan of it together with your photo ID to us. If accepted, the day of the party you gotta bring the original certificate and photo ID.
